Our team helps to close the gap in the continuum of care by providing compassionate and quality 服务 to enable patients to stay in the comfort of home during times of recuperation, 残疾, or chronic illness. Patient care is individualized and communicated throughout the team. Our experienced clinicians work with patients to develop a plan of care to help improve safety, 强度, 和平衡, and to maximize mobility.

The 家庭护理 team accepts referrals for evaluations and skilled home care, regardless of health care system or hospital, for all patients living in 纽约, 纽约海滩, 纽约港, Neddick角, 艾略特, 伯威克, North 伯威克, South 伯威克, Kennebunk, West Kennebunk, Kittery, Kittery Point, Ogunquit, 桑福德, and 井 (including Moody).
纽约 医院 家庭护理 provides 服务 7 days a week. An intake nurse is available to help with patient care referrals Monday through Friday, 8 a.m.–5 p.m. and on-call nurses are also available 24 hours a day.
